Read: G.O.Ms.No.25, dated 18.02.2019.

With reference to the above, it is noticed by the under signed that almost all the proposals for sanction of ex-gratia are being submitted are not in a full shape (enclosures being submitted only photo stat copies without attestation). Hence, all the Additional Project Co-ordinators, Samagra Shiksha in the state and they are requested to submit the proposals for sanction of Ex-gratia to the legal heir of the deceased employees who are worked under Samagra Shiksha and died in harness while in service as per the checklist as mentioned here under.
CHECK LIST :
The procedure to be followed as per the G.O.Ms.No.25 Fin (HR.1plg&Policy) Department dated 18-02-2019 by all the APCs in the State at the time of submission of proposal for sanction of Ex-Gratia is as mentioned hereunder.As per the para 5 of the G.O.Ms.No.25 is issued dated 18.02.2019 and in order to ope-rationalize the Ex-Gratia scheme, the following guidelines are to be adhered scrupulously.
1. The application seeking Ex-gratia shall be submitted by the Legal heir of the concerned deceased contract and Outsourcing employees with in a period of three (3) month from the date of demise to the unit/head of the office concerned.
2. After receipt of the application for sanction of Ex-gratia from the Legal heir, the Administrative Officer / Drawing and Disbursing Officer concerned has to conduct personal enquiry in the matter within fifteen (15) days and confirm the death.
3. The A.O. / D.D.O after through enquiry shall submit report to the unit/head of the office, confirming the death and recommends for sanction of Ex-gratia.
4. After receipt of the enquiry report, the Unit/Head of the Officer may sanction the Ex-gratia of Rs.5.00 Lakhs in case of accidental death and Rs.2.00 Lakhs in case of natural death, as the case may be
ENCLOSURES :
1) Death Certificate in Original
2) Legal heir in Original
3) Copy of enquiry report confirming the death of the employee.
4) Copy of FIR/Post-mortem report in case of accidental death (with attestation)
5) Copy of Letter / Representation of the Legal heir of the deceased within 3 months after the death of the deceased for sanction of Ex-gratia (with attestation)
6) Acknowledgement in Original towards payment of Funeral charges to Legal of heir of the deceased.
With the above documents a note file to be submitted to the District Collector and Chairman, SS for submission of the proposal to the office of the State Project Director, Samagra Shiksha, A.P, Amaravati for according permission to sanction Ex-gratia to the legal heir of the deceased. After getting approval from the District Collector and Chairman, SS a proposal should be submitted along with covering letter of the APC with the above documents and a copy of Note file approved by the District Collector and Chairman SS (with attestation) should be submitted to the office of the State Project Director, Samagra Shiksha, A.P., Amaravati for according permission to the Concerned APC for sanction of Ex-gratia to the Legal heir of the deceased with instructions to follow the guidelines as per the G.O.Ms.No.25 Fin (HR.1Plg&Policy) Department dated 18-02-2019.
Therefore, all the APCs, Samagra Shiksha in the State are requested to follow the above checklist without any deviation while submitting the proposals for sanction of Ex-gratia to the legal heir of the deceased employees who are worked under Samagra Shiksha and died in harness while inservice. The proposals which are not as per the check list, will be returned.
This has the approval of the State Project Director, Samagra Shiksha, Andhra Pradesh, Amaravati.
